When I get a COVID-19 vaccination, what information should I share?

When asked for your insurance information, tell your provider you have major medical insurance through Sidecar Health when receiving your COVID-19 vaccine(s) (including boosters). If needed, provide them with your Policy ID and Group ID (Sidecar Health). To find your Policy ID, go to your My coverage page in your account, click on the three-line menu, and select “Full benefits.” 

Note: You won’t be charged for COVID-19 vaccines, so you don’t need to worry about any costs or reimbursements. 

Your provider can bill Sidecar Health for COVID-19 vaccines directly. If your provider asks for a billing address, please provide the following:
